Please ring the practice on 9829 3033 during normal practice hours for an appointment. To streamline your arrival at the practice and save you time, please fill in the questionnaire here and submit. Once we have received your details, we will send you a confirmation of your appointment. Your appointment is not confirmed until we receive the questionnaire and we send you a confirmatory email or text.
You will be required to pay a $50 deposit to secure the appointment. Our staff will contact you to arrange for payment over the phone. This deposit will be deducted from your consultation fee when you attend for the consultation. The deposit is not refundable if you cancel with less than 48 hours notice.
